If the request is for a sermon

1. Your name and phone number or email address
2. Date and/or Title of Sermon
3. 7:30am or 10:45am service
4. Quantity of Compact Discs or DVDs
   

If the request is for a Bible Study class

1. Your name and phone number or email address
2. Date of lesson
3. Name of lecture series if known
4. Quantity of Compact Discs or DVDs

CDs are $5.00 each and DVDs are $10 each
